Pear Disease #854888

I have a Seckel pear tree & every season many of the pears have little dark holes on the skin & when I cut them open, have black areas. Black granules fall from the exterior holes. Can you identify what it is & what I can do to prevent/eliminate the problem?

Well, the out of focus images I think show that one of the fruit is cut in half with a worm near the center. Along with a hole on the outside with bits of debris it appears to be codling moth. A very common problem of apples and pears around here. Please look at the following two sites and see if it matches what you are seeing:
