Maryland doesn't have a soil testing lab anymore, but we have all the information that you need:

That page has a list of regional labs which can complete the analysis for you. You simply choose one, print out their forms, collect a good representative sample and mail it to them (a ziplock-type bag works fine). Be sure to let them know what you are growing (turf grass? vegetables? etc.) and that you are a homeowner.
