Clematis yellow leaves, not thriving, being eaten #766258

Please take a look at the pictures of my clematis plant. It is not thriving and looks like it is being eaten. I’m wondering if it might be a fungal disease or maybe being eaten by Nematodes or something else. Please let me know any ideas. Thanks!

Hi Laura. Did it just complete blooming? Can you tell me what kind of clematis it is and how long you have had it?
