Based on the photos.....the apple tree is planted in an unsuitable location for apples. It is on the edge of a wooded area, so its roots are competing with large, mature root systems of other trees. That could be why the roots are growing above the soil surface - they're growing on top of other roots. It's also competing for sunlight with the other trees, which is why it is so tall and skinny.
The white mold may be a trunk disease, but a sample would have to be sent to the Plant Disease Clinic for diagnosis.
