What's wrong with my apple tree? #653211

Can you please help identify this disease or insect damage on a Zestar apple tree? We use an orchard spray on a regularly scheduled basis as recommended. It doesn't quite look like apple scab or cedar rust. It appears like the outer green layer of the leaf has been chewed off, like a leaf miner might do, but we haven't seen any insects or caterpillars. Help!
