You didn't mention whether or not you have any other blueberry plants, but it is necessary to have at least one other bush of a different variety in order to get fruit production.  If you do have other blueberry bushes to provide cross pollination, it may be that there are inadequate insects to provide the proper pollination.  This could be the result of cloudy, rainy weather which deters bees and other insects from flying, or it could be the result of insecticides having been sprayed in the area.
If the bush is not producing any flowers, it may be the result of inadequate sunlight. 
In general, be sure that the pH of the soil is 4.5 to 5.5 and that you fertilize the plant only with ammonium sulfate.
