I have been putting my coffee grounds around/under our azaleas for a couple years or so. Is this a worthwhile thing to do, or a poor use of time?

A better way to use coffee grounds wouild be to add them to a compost pile or incorporate it into the upper 4' to 6' inches into the soil. Keep in mind azaleas do not like to have their shallow roots disturbed. Adding 2" of composted coffee grounds would be a good practice.
