What can cause 20 or 30 apples on a tree (with 200 apples on it) to develop one cavity on the surface, about 1/2 inch in diameter,mostly on a side. Then they start rotting and fall off. I have never seen this effect before and we have had apple trees giving apples for the past 42 years just north of Corvallis. There is no worm in the core, or worm in the small hole.

The spots look like they may have started as sunburn. Were the pots oriented to the sun during the hot part of the day? The holes look like bird pecks that targeted the sunburn spots.
