I think your plant may have Aerial blight (Phytophthora sp.). These plants are usually trouble free and not many diseases affect them. Without doing a microscopic examination aerial blight seems to be the best fit for the symptoms. It is present in the soil so it may have come along in the dirt and infected the plant when it was stressed.
